Case Manager

Hourly Rate: $18.50 - $22.50 - based on completed education level and years of experience.

As a member of the Integrated Care Team, the Case Manager participates in the development of the integrated comprehensive assessment and Individual Service Plan (ISP) and implements services to members in accordance with their plan and provides ongoing care and reviews progress. Coordinates with all involved healthcare, behavioral health and wellness providers and maintains current documentation on members’ progress in managing chronic medical and psychiatric conditions and assists the individual in achieving their recovery goals.

Minimum Qualifications:
• High School degree required, and minimum four (4) years of experience in behavioral health and SMI, OR
• Associates degree in related field with two (2) years of related experience OR
• Bachelor's degree in a related field with one (1) year of related experience.
• Case Managers are required to have the ability to transport members using their own vehicle. To meet this requirement, they must possess:
A valid Arizona (AZ) driver's license
A clean driving record
Current auto insurance and registration

• Must have a Level 1 fingerprint clearance card with no restrictions issued by Arizona Department of Public Safety OR be eligible to receive within eight (8) weeks of hire.
• Safety sensitive is any job that includes tasks or duties that the employer in good faith believes could affect the safety or health of the employee performing the task or others
• Bilingual (English/Spanish) preferred.
