Youth & Family Specialist - Phoenix Center:

The Youth & Family Specialist (YFS) maintains ongoing care-coordination services for children and families in need.

The YFS provides comprehensive support to children and their families through in-home and community-based interventions and education to address the client’s therapeutic goals. This may include parent education and skills training, behavior management training, the implementation of behavior-coaching plans, behavioral observations in various settings, consultations with school personnel, living skills training, and social skills training.

Duties include:

Facilitates Child and Family Team (CFT) meetings and provides home or community-based therapeutic and supportive services in accordance with agency procedures.
Assists children and families in obtaining community support, identifying resources, and resolving barriers.
Supports the most appropriate and least restrictive levels of care.
Works collaboratively with the Child & Family Team (CFT), medical, treatment, and management staff in implementing agency policies and procedures.

Qualifications:

-Must meet one of the following education/experience requirements:

High School Diploma/GED required and four (4) years of Behavioral Health experience
A.A. Degree and two (2) years of Behavioral Health experience
Bachelor’s Degree in Social Work, Counseling, Marriage and Family Therapy, Psychology or related field.
-Additional Qualifications:

Ability to adjust to changing and shifting priorities and duties for the benefit of the program.
Ability to work a 40-hour flexible schedule, including some nights and weekends.
Must possess strong verbal and written communication skills.
Ability to engage and work with diverse cultural groups.
Bilingual, Bicultural a plus.
Behavioral health experience; Inpatient/Residential experience; or Group Home/DDD/CPS in-home work experience preferred.
