**Responsibilities**

The Stroke Coordinator is responsible for coordinating and implementing services and activities related to the Stroke Services Program. This position functions as a liaison between physicians, nursing staff, allied healthcare professionals, patients and their families to coordinate and facilitate the care of the stroke patient throughout the continuum and to ensure the clinical care evidence-based guidelines are followed as appropriate.

The Stroke Program Coordinator participates in program development as it relates to the Primary Stroke Center. This includes staff and physician education, community education, marketing, planning, developing and implementing Stroke Center initiatives including quality, safety service and efficiency strategies into the plans policies, and organizational processes that affect the Stroke Center’s operation, strategic direction and certification.

+ Acts as liaison to the community by providing stroke educational seminars and referrals.lectures, interviews and presentations to community audiences. Participates in county-wide stroke awareness and education programs.

+ Reviews literature, research and new community practice to identify changes in practice. Serves on committees as appropriate to provide clinical and theoretical expertise in stroke and clinical documentation

+ Demonstrates Leadership abilities in: Human Resource management, decision-making, problem solving, critical thinking, managing multiple tasks in a dynamic environment. Program and project management skills. Consistently demonstrates positive interpersonal skills with patients, families, staff, and physicians. Demonstrates knowledge of principles and methods of excellence in clinical practice, community, hospital and departmental nursing standards, safety, security, infection control practices and protocols, regulations governing the hospital (Title 22, OBRA, Joint Commission, CMS, CDPH).

+ LANGUAGE SKILLS: Able to read, analyze, and interpret professional journals, technical procedures, guidelines and regulations. Ability to document assessment, treatment information, and other health related data as well as complete quality reports. Ability to effectively articulate information or data to multiple audiences with varying level of knowledge and expertise.

**Qualifications**

Per policy, current employees are not eligible.

**REQUIREMENT:**

+ Clinical experience in care for stroke and neuroscience patients is highly desirable. Three to five years experience in teaching and/or clinical program development and coordination is desirable. Experience in data collection and statistical analysis preferred.

+ Graduate of an accredited school of nursing with BSN, MSN or Certified Nurse Practitioner

+ Current California RN License.

+ Completion of annual safety training. Completes and maintains required hospital specific training.

**Pay Range**

$56.99 - $82.64 /hour
