Banner University Medical Center Transplant Institute is Arizona’s most experienced transplant center, providing comprehensive care to adult patients in pre and post-transplant acute clinics for heart, lung, liver, and kidney/pancreas transplant, as well as, long term follow up on the transplant recipient. As part of the Banner Health Academic Medical Division we offer Transplant Services in Phoenix and Tucson, AZ. Our Transplant Institute prides itself on delivering compassionate, quality care to our patients and families leading to successful, optimal outcomes. Performing over 450 transplants per year, our programs are poised for continued growth!

Hours for this position are Monday-Friday 8:30 a.m.-4:30 p.m. Work location will be at Banner University Medical Center in Tucson.

POSITION SUMMARY

This position serves as first point of contact for referral of patients for potential transplant evaluation for referring physicians, their office staff, hospital financial administration, and for the patient. Coordinates the evaluation process, provides education to the patient and family about the process and participates in outreach activities and marketing projects which result in strengthening community relations and increased referral volume.

CORE FUNCTIONS

1. Coordinates the referral and evaluation of potential transplant recipients. Ensures that protocols are followed and all necessary tests are completed prior to listing patient for transplantation. Assists the Transplant Physician in obtaining the initial patient referral. Acts as a liaison between the hospital financial administration group, the patient, transplantation services and the referring physician in the patient's pre-evaluation phase. Care of the patient during emergency referrals will include: Organization of air/ground transport, possible artificial devices, as well as emergent testing. Coordinates patient care during the evaluation phase. Secures physician orders, provides patient and family education, and organization of interdisciplinary testing per protocol. Gathers and summarizes patient test data. Presents patient at transplant conference. Communicates results of tests as well as team's decision to hospital financial department, referring physician, and patient's insurance carrier. Arranges for meeting with patient to review data. Provides ongoing assessment of waiting patients' candidacy. Notifies the United Network Organ Sharing (UNOS) service that the patient is an acceptable transplant candidate. Additionally, provides UNOS with the necessary information to list the patient for transplantation.

2. Coordinates activities at the time of organ transplantation. This includes activities related to donor procurement and recipient transplantation. Receives initial donor/organ information from United Network for Organ Sharing (UNOS). Communicates donor/recipient information to surgeon and procurement team. Organizes patient admission. Calls patient, arranges admission, and notifies departments per transplant protocol.

3. Models effective and positive communication skills with physicians, health care professionals, support staff, and patients, including accurate documentation. Communicates with referring physicians of transplant recipients. Interfaces with ancillary personnel and acts as a liaison between members of the transplant team and with patient and family members. Displays courteous and caring behavior when communicating with patients. Is tactful in difficult patient circumstances; maintains privacy and confidentiality of information relative to patient care. Participates and organizes both the Patient Selection Committee meetings and Patient Care Committee meetings.

4. Performs administrative and education activities for transplant patients and hospital staff. Develops transplant education materials for patients and staff. Assesses the patient and family for educational needs and learning abilities based on age appropriateness. Provides patient/family education with individualized instruction and written material. Documents all patient education activities in the medical records. Provides and attends community education programs. Educates potential recipient family and explains the transplantation process. Educates, instructs, and counsels the pre-transplant and post-transplant patient. Participates in the education of the hospital staff regarding transplantation. Participates in the administrative aspects of the Transplant Service especially in providing information for insurance companies' request for protocols.

5. Continually evaluates program and redefines objectives and/or goals. Works throughout evaluation period to achieve goals. Accomplishes goals within the agreed time period. Participates in defining the goals of the department. Works toward the achievement of department goals.

6. Works independently to provide excellent patient care. Internal customers include patients, staff, and physicians. External customers may include patient family and community organizations, and the United Network Organ Sharing (UNOS) service.

MINIMUM QUALIFICATIONS

Knowledge and understanding of a healthcare setting typically obtained by an associate degree in a healthcare related field.

A minimum three years' experience in a healthcare setting is required. Requires thorough understanding of diagnostic tests utilized for transplant evaluation and related scheduling procedures, ability to present patient case in selection committee and ability to provide education to patients about the necessary tests, procedure, and related processes. Also requires excellent organizational and communication skills; ability to multi-task; ability to work under pressure and meet deadlines; ability to demonstrate project management skills.

PREFERRED QUALIFICATIONS

Five years' experience in a healthcare setting preferred.

Experience with coordinating patient care and education preferred. Experience working as a Healthcare Technician preferred.

Additional related education and/or experience preferred.
