SUMMARY
Provides counseling, supervision, and casework services to clients in a detoxification facility, acute psychiatric health care residential facility, in-home, or day treatment setting.
DUTIES
Provides counseling and supervision of clients in individual and group settings as part of a multi-disciplinary team to achieve treatment goals.
Maintains client records including evaluation, follow-up, and closure activity according to established guidelines and procedures.
Provides supportive counseling and training in daily living skills such as personal hygiene, nutrition, budgeting, and food preparation and purchase.
Intervenes in crisis situations and attempts to avert potential crises.
Observes and reports nursing functions such as medication effects, nutrition, infection control,etc.
Directs, participates, and assists clients in planning and carrying out activities, and evaluates client activities.
Acts as advocate for clients and family in dealing with other agencies and the public.
Transports clients in agency vehicles as necessary.
Consults with, makes referrals to, and follows-up with other community agencies to provide needed client services.
Implements and maintains safety standards for clients and staff.
Attends and participates in in-service training and staff meetings.
May assist in client evaluations and in planning and implementing behavioral treatment plans.
May assist in orientation and training of new staff.
May evaluate and assess job skills and work potential.
May assist in the routine maintenance of equipment and vehicles.
Assists in public relations and community education activities.
May assist in the initial interview and intake of clients.
May provide outreach and aftercare services on-site.
Coordinate all care and services required to enhance and facilitate the optimal physical and behavioral health wellness of persons served within a trauma-informed care framework this includes:
facilitating communication between medical and behavioral health practitioners/systems;
assisting with scheduling of and compliance with preventative and specialty physical health interventions;
integrating treatment interventions to address both the physical and behavioral health needs of the person/family concurrently;
assist persons/families served with education and resources outlining the interplay of physical and behavioral health issues on overall wellness and quality of life.

REQUIREMENTS
High School diploma or GED
Individuals must be 21 years of age or older
